Series Purpose: This ECHO series will support healthcare professionals serving rural Kansans to bridge gaps in health care and build healthier communities. The series will introduce learners to the barriers many Kansans face when seeking health care, tools and strategies that work to troubleshoot and mitigate health disparities in Kansas communities, and the values of employing community health workers, nurse navigators, and other support staff to improve health outcomes. Learners will be connected to vital resources such as data sources, screening tools, information on building sustainability for community health workers, and practical strategies for combating health disparities.

Target Audience: All physicians, advanced practice clinicians, pharmacists, nurses, behavioral health clinicians, and community health workers are invited to join this ECHO.
