How to Submit a Referral with eligibilityLink

The way you submit a referral request depends on the member’s plan. You can use eligibilityLink to determine the right tool to use. View Online Referral Guide for additional support. Here’s how:

  1. Sign in to Link by clicking on the Link button in the top right corner of this page.
  2. Look up the member in the eligibilityLink app.
  3. Click on the Referrals hyperlink - see Figure 1 below.
  4. Follow the prompts and complete the menu options to submit your referral.

How to Check the Status of a Referral

Once the referral is submitted, use the information below to choose the right tool for checking referral status based on member plan type. If you need to make changes to a referral, please submit a new referral request.

Use for Member Plan Type:

  • UnitedHealthcare Commercial
  • UnitedHealthcare Oxford members with a numeric (not alpha-numeric) Group Number
  • UnitedHealthcare Medicare
  • UnitedHealthcare Community Plan (all states except Florida and Rhode Island)

To access the tool, sign in and select Referral Status from the Notifications/Prior Authorizations menu.

  • UnitedHealthcare Community Plan of Florida
  • UnitedHealthcare of Florida Dual Special Needs Plan (DSNP) — starting Jan. 1, 2018
  • UnitedHealthcare Community Plan of Rhode Island - on or after Jan. 15, 2018
