The VFC Program provides you with all routine vaccines for your Medicaid eligible patients, ages 0 through 18, at no cost. This is a federally funded program offered through the state of Michigan.
This program helps prioritize preventive care for Medicaid-eligible patients, reduce disparities and safeguard communities from preventable diseases. The VFC program works to ensure no child misses out on essential vaccinations due to barriers to care.
Any health care provider authorized by their state to prescribe vaccines can be a VFC provider. To enroll:
Practices must fax a copy of their current, non-expired VFC certification, along with a list of the practice address(es) covered by the certification, to UnitedHealthcare Community Plan of Michigan at 844-304-2840.
