A care provider that formally joins to participate in certain incentive programs will receive a payment from UnitedHealthcare if the requirements described in these Terms and Conditions are met. Specific program information follows.

These Medical Condition Assessment Incentive Program for Out of Network Providers1 Terms and Conditions (“Terms and Conditions”) govern the Medical Condition Assessment Incentive Program (“MCAIP”). As a precondition for the Provider to participate in the MCAIP Program pursuant to these Terms and Conditions and to be eligible for the payments described below, one of the following must have occurred: (a) UnitedHealthcare (“United”) presented an MCAIP Program Participation Acknowledgement (“Acknowledgment”) to Provider and Provider signed and returned the Acknowledgement to United in accordance with the deadline established by United, or (b) United notified Provider of Provider’s enrollment in the MCAIP Program via a unilateral amendment to Provider’s participation agreement with United.
